If you need any advice or information not covered by this leaflet, please ask the Day Care staff. It has been suggested that you may benefit from attending Day Care at St Ann's Hospice. Both Little Hulton and Heald Green Day Care is part of the Day Therapy Service, which also includes Supportive Outpatients and Medical Palliative Care Clinics. This leaflet outlines some information about us which we hope will help you decide whether or not Day Care is suitable for you. Your day Attending Day Care will give you a day away from home and offers you an opportunity to share any worries you may have with the staff. It also allows a rest day for the person who may be looking after you. We can also help you gain more confidence at home by assessing your personal needs, helping you overcome them wherever possible. Come and See You are welcome to come and have a look around, either alone or with the people caring for you. This should be organised beforehand, either by contacting your Macmillan or District Nurse or directly with us at Day Care. Telephone: HEALD GREEN: 0161 437 8136 (Monday to Friday, 9.00am to 4.00pm) Initially you may be invited to attend one day each week. This will be reviewed within 12 weeks of your first attendance. Daily Routine Although everyone has different needs, we do have a general routine. People usually arrive around 10.00am and will be offered tea or coffee. Lunch is served at mid-day and at approximately 2.30pm we have afternoon tea/coffee and cake. People leave for home at 3.00pm. Please remember to bring with you any items that you might require: Handbag House Keys Spectacles Toiletries Medicines etc Your medicines On your FIRST attendance, please bring with you ALL YOUR TABLETS AND MEDICINES. The Day Care staff will wish to check these with you and ask about how you have been and how you are feeling now. Every time you attend Day Care, please bring with you any tablets or medicines you require during the day. You will be responsible for remembering your tablets and for their safe-keeping. If you need the Day Care staff to HELP give you any medicines or tablets, you will need to: EITHER 1. bring the tablets or medicines in the original container - clearly marked OR 2. bring the tablets in the Dosette box which has been filled by the Pharmacist Your GP While attending Day Care, you will remain under the care of your GP and the Hospital Consultant. The Hospice Doctor may suggest changes in your tablets or medicines but, before any change is made, your GP will be contacted who will continue to provide all your prescriptions. Should you find you are unwell between visits to Day Care, you should contact your GP. If you have a District and/or Macmillan Nurse, he or she will be able to advise you. In the event of a patient experiencing a sudden, unexpected collapse, active supportive care will continue to ensure the patient is comfortable and free from all pain and distress. Active resuscitative care - that is, treatment to try and reverse the causes of sudden, unexpected collapse - may not be appropriate for patients at St Ann's Hospice. If a patient is attending St Ann's Day Care for whom such active resuscitative care is thought to be appropriate, then this will be discussed with the patient on their attendance. Transport We have an excellent team of hospice volunteer drivers who, if required, collect you in the morning and take you home when day care is over. If getting in and out of a car is difficult, we have a hospice minibus, with accompanying nurse, to help you. The minibus has a hydraulic lift, so even if you use a wheelchair it is not a problem. All our transport is free of charge. PLEASE PHONE the hospice (before 8.45am) to inform us if, for some reason, you are unable to attend.
