How to refer a patient

If you feel one of your patients may benefit from the services provided by St Ann’s Hospice, this page will give you a quick guide of how to refer them to us.

If you are unsure which services may best suited to your patient, please contact us and we would be happy to discuss it with you, or take a look at the ‘our care’ section which gives comprehensive information on the services St Ann’s Hospice provides.

Inpatient care

Referrals can be made through a healthcare professional at a hospital, such as a consultant or a doctor, a GP or palliative care specialist nurse who will need to complete and submit our hospice referral form.

Day care

Referral can be made by general practitioners, district nurses, hospital medical teams, community or hospital specialist palliative teams and social care teams. Referrals can also be initiated by the patient, but this will be in consultation with the above health and social care specialists. Referral is by completion of a hospice referral form.

Supportive outpatients’ services

Access may be at the point of diagnosis, during treatment, following treatment, at times of disease recurrence or at any other key point in the patient’s illness. The sooner someone is referred to us, the sooner we will be able to help. Referral can be made by any health and social care professional over the telephone or by completion of a hospice referral form. Individuals wishing to access the service may self-refer by telephone or in person.

Community support

Hospice@Home teams (Salford and Trafford)
Patients and/or carers can ask their doctor, district nurse or specialist palliative care nurse to contact the respite team on 0161 702 5405.

Community specialist palliative care nurse team (Salford)
Referrals can be made by any health and social care professional and can also be initiated by the patient, family or carer (in consultation with relevant health and social care professionals). Referrals can be discussed with any member of the community specialist palliative care team on 0161 702 5406 (Monday to Friday 9:30am – 4:30pm)

Outreach Complementary Therapy Service (central and parts of north Manchester)

Telephone referrals are accepted from district nurses, community specialist palliative care nurses and general practitioners. For more information please contact the Neil Cliffe Centre on 0161 291 2912.
