Advance Care Planning (ACP)

Develop the knowledge and skills to feel comfortable and confident talking to people about Advance Care Planning. You'll finish the training with a clear structure to facilitate the conversation.

  • Online: delivered on a virtual platform
  • Length: 1 hour 30 minutes (2 – 3.30pm)
  • Suitable for: health and care professionals in hospices, hospitals, nursing or care homes who are supporting people who would benefit from planning their future care
  • Price: £50 for three sessions

 

Course details

Session 1: Importance of ACP 

Join us to discover why Advance Care Planning is important, discuss what can be included and assess whether the person has mental capacity to discuss their future care plan.

Session 2: Communication skills for ACP 

Uncover the skills you need during an Advance Care Planning conversation. You will practice facilitating a conversation using the Simple Skills Communication model.

Session 3: Documenting and Sharing 

No more hours of frustration trying to piece together how a person’s wishes and preferences can be documented and shared.  You’ll take away all the pressure out of helping a person to write their advanced care plan. Take away the knowledge of what can be included in an advanced care plan and the how, when and why of sharing.
