About the course:
This programme is funded by Health Education England North West, designed by the North West Coast Learning Collaborative and delivered by Education Hubs throughout the North West of England to make Advance Care Planning a daily reality for individuals.

The training is divided into 4 distinct sections: Think about it, Talk about it, Record it and Share it.

The course covers the importance of advanced care planning, communication skills, the principles of the Mental Capacity Act and how this essential information is effectively recorded and shared.
