We’re looking for a senior clinical decision maker who can provide advice, clinical, operational and strategic leadership to the community specialist palliative care team.

The Role

The Clinical Fellow will:

  • Be responsible for maintenance of the highest possible standard of medical care for patients
  • Assist in the daily management of patients on the inpatient unit including practical procedures such as venepuncture and intravenous infusions
  • Undertake initial and ongoing assessments on the inpatient unit, using the electronic patient record with senior doctor support and supervision
  • Participate in regular consultant led ward rounds
  • Support may also be provided to the Being You Centre, where medical assessments are required
  • Work closely with members of the Multi-Disciplinary Team enabling a holistic approach to patient care
  • Participate in education, training, audit , quality improvement and research activities
  • Undertake appropriate Continuing Professional Development, appraisal and revalidation activities
  • Liaise with, and provide advice to members of the Primary Healthcare Team concerning admissions and the 24-hour advice line
  • Be involved in the provision of 1 in 5 first on call at Heald Green with Consultant second on-call. First on-call will involve non-resident on-call in the evenings and a ward round at the weekend

Requirements

Essential Criteria

Be fully registered with the GMC

Have an MBBS or equivalent qualification

Have a minimum of 2 years’ experience since qualifying / graduation

Demonstrate acquisition of the level of knowledge and skills necessary for the completion of F2 within 2 years of intended start date

Have a breadth of clinical experience (e.g. oncology, elderly care, general medical specialties)

The Clinical Fellow will be able to:

  • communicate clearly and sensitively with patients, families and colleagues
  • Be sensitive to patients’ spiritual and psychological needs
  • Understand the importance of audit, research and quality improvement in the continuing development of clinical practice
  • Show awareness of their own limitations
  • Teach undergraduates and other health professionals
  • Demonstrate organisational skills and an ability to prioritise
  • Have multidisciplinary team working skills, a specific interest in palliative medicine and a desire to develop skills further
  • Demonstrate a breadth of experience in clinical practice
  • Ideally have had personal involvement in audit, research or quality improvement
  • Demonstrate a knowledge of the importance of clinical governance
  • Show an understanding of voluntary sector funding and organization
  • Attend the hospice within 45-60 minutes when on call
  • Use a non-judgemental approach to patients and colleagues
  • Show IT skills to manage the electronic patient record, clinical systems and Microsoft Office
  • Meet professional health requirements

Job Summary

We have an established medical team with links to Salford Royal, Manchester, and Stockport Foundation Trusts.  We provide medical care to the 18 beds at Heald Green and 18 beds at Little Hulton but also support the Being You Centre, Outpatient Services and a 24-hour advice line.

We are developing our medical workforce to meet the demands of a modern specialist palliative care unit and have an exciting opportunity to appoint to a Clinical Fellow post across the organisation.

This post will play an integral role in delivering holistic care to our patients on the inpatient unit and supporting in audit and quality improvement activities.

This post is primarily based at the Heald Green site. There may be occasions where you are required to work cross-site.

The Role

The Staff Nurse will assess, plan, evaluate and deliver all relevant aspects of specialist palliative and supportive patient care in a caring, compassionate and sensitive manner.

The Staff Nurse plays an integral part of the multi-disciplinary team and has strong working relationships with other healthcare providers.

The Staff Nurse assists in the management and organisation of work as required, ensuring effective communication and the maintenance of high standards of quality patient care on a shift basis.

The Staff Nurse plays a key role in the supervision and assessment of all learners and unregistered staff, providing a teaching role and acting as a named mentor.

St Ann’s delivers a service across three sites. Whilst you will have a specific base, there may be occasions where you are required to provide cross-site / service cover.

The Person

The Staff Nurse will:

Take overall responsibility for the coordination and safe effective management of the Inpatient Unit (IPU) on a shift-by-shift basis as delegated by the IPU Manager / Sister / Charge Nurse.

Undertake comprehensive health care needs assessment of patients, reassessing as appropriate.

Collaborate with other health care professionals in the delivery of high standards of effective holistic care.

Lead on the delivery of specialist palliative and supportive care, ensuring individual care needs are assessed, planned, implemented and evaluated.

Use professional judgement when assessing patient needs / problems and investigations requiring analysis and intervention.

Organise own time and that of junior learners.

Promote the safeguarding of vulnerable patients in line with national and local policy.

Ensure that patient information is communicated to the appropriate professional(s) of the multi-disciplinary team.

Practice in accordance with the NMC Code of Conduct, the hospice values, policies and procedures and professional guidelines in order that a safe and quality service is provided.

Communicate effectively with dignity and respect to all patients, relatives, staff and others giving consideration to potential barriers to understanding.

Work towards the achievement of the shared goals of the patient and multidisciplinary team by collaboration to ensure that best practice is achieved utilising both clinical bench marking and Evidence Based Practice to achieve this.

Undertake practice sensitive to the individualised needs of all patients.

Administration of medicines and treatments in line with NMC and hospice policy.

Be actively involved with all aspects of admission and discharge, engaging with other agencies and community teams to ensure a safe, individualised approach based on the assessment of needs.

Undertake clinical skills with dexterity and accuracy to improve the patient experience and journey (e.g. venipuncture, cannulation, etc.)

Be competent in the correct use of all equipment used in the clinical setting in accordance with instruction and departmental procedures, reporting any faults as necessary.

Maintain accurate, contemporaneous electronic patient records in line with hospice policy and NMC Code of Conduct.

Maintain own CPD in accordance with requirements and contribute to formulation of own objectives and personal development plan.

In conjunction with the IPU Manager / Sister / Charge Nurse, contribute to the delivery of orientation programmes and induction for new starters and learners.

Actively contribute and participate in the hospice clinical governance agenda.

Act as a link nurse and resource for other nursing staff for a specific area of patient care.

  • The physiotherapist will be a key member of the multidisciplinary team; they will be a role model for clinical staff, demonstrating a professional and proactive approach to providing rehabilitative advice and support to patients with life limiting illnesses.
  • The physiotherapist will work with rehabilitation team colleagues to deliver a physiotherapy service at the Little Hulton Site. They will need to be able to prioritise and work efficiently to ensure the complex needs of in patients and outpatients are met in a timely manner.
  • The physiotherapist will actively participate in developing cross site physiotherapy services at the hospice.
  • The physiotherapist will be aware of and deliver services in line with best practice guidelines and the requirements of the care quality commission in order to ensure the aims and objectives of the organisation are met.

Band 7

  • The post holder will be responsible for contributing to the effective delivery of physiotherapy services to inpatients and outpatients at the hospice.
  • The postholder will carry out clinical duties in accordance with hospice policies, procedures and standards.
  • The post holder must act in accordance with professional Codes of Conduct at all times and will contribute and work collaboratively with the wider MDT in line with the clinical and hospice strategy, values and business objectives.
  • The post holder will deliver specialist clinical services as a Physiotherapist.
  • St Ann’s delivers a service across two sites and the post holder will complete physiotherapy interventions at both sites. There will therefore be occasions where you are required to travel and provide cross-site cover.
