Making a referral

If you feel someone may benefit from the services provided by St Ann’s, this page will give you a quick guide to referring them to us.

All health and social care professionals can make a referral to St Ann’s services.

Patients, families and carers can seek a referral directly for The Being You Centre, and also for the Community Specialist Palliative Care Team in Salford (in consultation with relevant health and social care professionals).

For more information about our referral process, please download the referral handbook below. You can download the relevant referral forms at the bottom of this page (once the form has downloaded, save it to your computer before filling it out).

Inpatient care

Referrals can be made through a healthcare professional at a hospital, such as a consultant or a doctor, a GP or palliative care specialist nurse who will need to complete and submit our Referral Form (which can be downloaded at the bottom of this page).

  • Heald Green 0161 437 8136
