Making a referral

If you feel someone may benefit from the services provided by St Ann’s, this page will give you a quick guide to referring them to us.

All health and social care professionals can make a referral. Also, patients, families and friends can seek a referral directly. For more information about our referral process please download the Referral Handbook at the bottom of this page.

Inpatient care

Referrals can be made through a healthcare professional at a hospital, such as a consultant or a doctor, a GP or palliative care specialist nurse who will need to complete and submit our Referral Form (which can be downloaded at the bottom of this page).

Referral can be made by GPs, district nurses, hospital medical teams, community or hospital specialist palliative teams and social care teams. Referrals can also be initiated by the patient, but this will be in consultation with the above health and social care specialists. Referral is by completion of a Referral Form (which can be downloaded at the bottom of this page). We also run a day care drop-in service at our Heald Green site.
